FROM THE DESK OF THE SCHOOL NURSE
A review of electronic student health records has prompted the school nurse(s) to post these reminders to families. Students with certain health concerns are required to have current health forms on file in order to meet their needs at school. Health forms must be renewed each school year. The following are common school health concerns:
- Medication Administration at School
- Asthma (especially with inhaler and/or nebulizer use)
- Food Allergies and Anaphylaxis (allergies that require Epi-pens)
- Health Concerns that may require an Emergency Action Plan (bleeding disorders, etc.)
Please review each student's electronic PowerSchool record for accurate health data. If you feel that your student no longer has a health concern or has developed a new health concern, please make changes to your student's PowerSchool profile under health concerns.
If you have questions regarding a health concern or about the health forms please contact the nurse's office by phone at 641-664-2200 ext. 1120 or email.
ATTENTION: Emergency contact information is important to include when registering your student each year. Please ensure that you provide emergency contacts that can respond to school in a timely manner in the event that we are unable to reach you and your student becomes ill or injured at school.
