2019 Community Health Needs Survey

Davis County Hospital has asked us for our help in spreading the word on the 2019 Community Health Needs Survey. The response they get from Davis County residents helps them secure grants and funding to continue supporting our community. Please take a moment and complete the survey linked below when you have time.

No one knows your community better than you. Davis County Public Health and Davis County Hospital are working together to complete the 2019 Community Health Needs Assessment (CHNA), and they need your feedback. We invite you to help improve the care and services in our community by visiting the link below and answering a few short questions. Please answer honestly; your responses will be kept confidential.  

Data from this CHNA survey will help health care providers and community organizations improve the overall health and wellness of our community, and identify community needs, establish priorities for programs and services, and inform strategies to address gaps between critical needs and services. A great example of how important the results of the assessment are that our local Nest Program was able to secure a large portion of their grant funding by utilizing results from the 2016 Community Health Needs Assessment.

All Davis County, Iowa residents and business leaders are encouraged to participate. The deadline for survey collection is October 15, 2018.
